- Supporting a comprehensive bone care plan: Combines medication infusion, appropriate calcium/vitamin D supplementation, weight-bearing exercise, fall prevention, nutritional adjustments, and routine bone mineral density monitoring.
Indications
The Bone Protection package may be considered by a physician for the following groups:
- Postmenopausal women with osteoporosis or high fracture risk.
- Older adults with reduced bone mineral density, osteoporosis, or a history of low-trauma fractures.
- Adult males with osteoporosis or high fracture risk.
- Individuals on long-term glucocorticoid therapy with osteoporosis risk or documented evidence of reduced bone mineral density.
- Individuals requiring prevention of recurrent fractures following a low-trauma hip fracture, after initial stabilization and when evaluated as eligible for infusion by a physician.
- Clients whose DEXA bone density scan results or fracture risk assessment indicate a need for antiresorptive therapy.
Pre-Procedure Evaluation
Prior to prescription, the physician should consider evaluating:
- DEXA bone mineral density testing at the lumbar spine, femoral neck, or total hip.
- Fracture risk assessment based on age, history of fractures, falls, menopausal status, BMI, family history, corticosteroid use, smoking, alcohol consumption, and underlying medical conditions.
- Serum creatinine/eGFR testing or creatinine clearance calculation to evaluate renal function.
- Serum calcium, vitamin D, phosphorus, magnesium levels, and mineral metabolism disorders if necessary.
- Oral and dental health evaluation, particularly in individuals with periodontal disease, loose teeth, planned tooth extraction/implants, or risk factors for osteonecrosis of the jaw (ONJ).
- Current medication review, particularly diuretics, drugs affecting renal function, corticosteroids, or other osteoporosis medications.
Precautions
- Administer only with a physician’s prescription/indication.
- Do not administer in the presence of uncorrected hypocalcemia.
- Do not use in patients with severe renal impairment or creatinine clearance below the allowable clinical threshold.
- Do not use in pregnant or breastfeeding women.
- Ensure adequate hydration prior to infusion, especially in elderly patients or those taking diuretics.
- Ensure adequate calcium and vitamin D supplementation as directed by a physician.
- Post-infusion acute-phase reactions may occur, such as fever, myalgia, arthralgia, headache, fatigue, or flu-like symptoms, typically within the first few days.
- Patients must notify a physician immediately if experiencing jaw pain, loose teeth, delayed oral wound healing, thigh/groin pain, muscle spasms, circumoral numbness, numbness in extremities, or signs of hypocalcemia.
- Do not co-administer with other medications containing zoledronic acid or other bisphosphonates without explicit medical indication.
